Medical Evacuation File Checklist (Free PDF)

A complete file lets the right aircraft and the right medical team be chosen on the first try. We have turned our own working method into a one-page checklist, for assistance teams, insurers, employers and families.

What the checklist covers
- At the first call: the four things to have at hand.
- 1. Identity and administration, including the guarantee or the payer.
- 2. Medical summary and infection status.
- 3. Imaging and tests, in a format the receiving team can open.
- 4. Opinion on fitness to travel.
- 5. Needs on board: stretcher, oxygen, ventilation, specialist team, weight and height.
- 6. Receiving end: hospital, unit, physician and bed.
- 7. Contacts and constraints.
- 8. Consent and confidentiality.
How to use it
Print it or keep it on screen, and tick the items as you gather them. An incomplete file should not delay the first call: we start with what is available and complete it as the case progresses. Our coordinating physician reads the file, tells you what is missing and contacts the treating hospital directly.
The checklist is available in English, French and Turkish. It gives general information and does not replace the opinion of the treating physician or of the coordinating physician.

General information only — not personalized medical, legal or insurance advice. Your insurance contract and the treating physician's opinion prevail in your situation.
A medical situation abroad can't wait.
